Patient Information

​Please arrive 30 minutes prior to your appointment to complete the forms listed below. If possible, please print and complete these forms prior to your appointment date and bring them with you to your visit:

Please bring copies of previous medical records and test results for the doctor to review and retain. This includes all relevant CT and MRI pictures (these will be returned to you at the end of your visit).

Our center accepts Medicare and many major health insurances. Patients are responsible for verifying our participation with their specific insurance carrier prior to scheduling an appointment. Thank you!

Patients are responsible for obtaining any necessary referrals prior to their appointment. Payment is expected at the time of your visit. We accept cash (exact amount for copayment is appreciated), check, travelers check, money order, Visa, MasterCard, American Express, and Discover.

With few exceptions, our doctors run on schedule. Please be on-time for your appointment, and be sure to allow time for transportation, parking, and our patient registration process. If you arrive late, we will do our best to accommodate you, but it may be necessary to reschedule your appointment. If you need to cancel an appointment, please contact our office at least 48 hours in advance. This allows us to schedule other patients who are waiting for an appointment.

Street parking and a parking garage at the corner of Fort Washington and 165th Street are available. We are sorry but this parking garage does not allow us to validate patient parking.

International Patients
